If your establishment keeps records of only the hours paid or if you have employees who are not paid by the hour, please estimate the hours that the employees actually worked. Certain Treatment Not Allowed Expenses will not be paid for treatment by a physical therapist, masseur or masseuse, or pain clinic unless the treatment is ordered by a doctor or unless the employer or insurance company specifically agrees in advance to pay for such treatment.
